Include two new fields in the opt-in form:

• Website address (with validation)
• Message box (Input text area)

The current Optin Form only supports name & email fields. 

Please include option to add more fields like phone number, website, simple text input fields with custom field name support.

I know there are many form builder plugins available but it will be better to have some essential form fields support builtin with WPfunnels so it doesn't have to rely much on any third party plugins.

It will also be great if we can export the submissions including field label in csv file format to use it elsewhere .
